Applications

In order to participate in one of our programs you must complete an application. There are four applications, so please make sure you fill out the correct application for the program you wish to do.

Chaminade students who wish to participate in our programs are also required to use this application instead.

 

Remember to read the guidelines provided because if all the required materials are not turned in your application will be deemed incomplete.

NOTE: The $20 application fee must be a check or money order. No cash or credit is accepted.
